community health improvement plan cover page
We are excited to announce the release of the 2023 Community Health Improvement Plan (CHIP) for the Scott County and Rock Island County Health Departments! Due to the COVID-19 pandemic interrupting the implementation of the 2019 CHIP, community partners decided to transition several strategies and activities to this CHIP for all priority areas: mental health, nutrition, physical activity, and weight, and access to healthcare. This plan will help guide the collaborative efforts taking place over the next three years to improve the health of our community.
