A Guide to Understanding Your EOB
After you visit a provider, your health plan may send you an explanation of benefits (EOB).  It gives you important details about what your plan covers and what you may owe. 

Here's what to look for:

  • The name of the member/patient will be displayed in the upper right hand corner.
  • A quick snapshot of the claim will be listed in the Claims Summary section.  In this section you will see how much your provider billed your health plan, any plan discounts, how much your plan paid and how much you may owe.
  • In the Claims Detail section you will find a detailed list of the services performed.
    • You will also see information regarding your deductible, copay, coinsurance and the total you may owe your provider.
    • This section also includes a breakdown of how much your plan paid for each service.
  • The Notes section may also include more details on why a claim was- or was not- paid.
  • A Summary of how much you had paid toward your deductible and out-of-pocket maximum at the time the claim was processed will be listed in the Account Summary section.
  • Your EOB may also include Definitions of Key Terms that may help you better understand your EOB.

What to do next:  Check on your claims and review your EOBS by logging in to your www.myuhc.com account.
