Genesis Occupational Health will be onsite to administer the flu vaccine on the following dates:

• WEDNESDAY, October 2nd from 7:30 a.m. - 10:00 a.m. on the 1st floor Board Room of the Administration Center.
• THURSDAY, October 3rd from 6:30 a.m. - 7:30 a.m. at Secondary Roads  (SIGN UP directly with Tara at Roads)
• FRIDAY, October 4th from 2:30 p.m. - 3:30 p.m. at SECC
• MEDIC flu shots will administered by Medic staff

PLEASE SIGN UP BELOW TO RECEIVE YOUR VACCINE AT THE LOCATION MOST CONVENIENT FOR YOU!  Please print and fill out the attached influenza consent form, and bring it with you.

**Individuals employed by Scott County are invited to obtain the vaccine on-site.  A spouse and adult children covered on your Scott County group health insurance plan are welcome to sign up and receive the vaccine on-site as well. 
The vaccine is for adults, 18+ only.  Pregnant participants will need prior authorization from their doctor or OBGYN.

If you are unable to obtain the flu vaccine during one of our onsite clinics, please feel free to visit your primary care provider or local pharmacy.  Please note it’s important that the pharmacy runs your insurance through medical coverage* and not pharmaceutical coverage. The Flu vaccine is covered at the following pharmacies:

• Hy-Vee
• Walgreens
• Walmart
• Costco
• Sam’s Club

*CVS - to be run under pharmaceutical coverage for it to be covered.

You may also visit any of the Occupational Health Clinics after October 1, no appointment is necessary at the Bettendorf, Davenport, or Moline Locations.  (Addresses are on the Steps to Stop the Flu Attached Flyer)
